C loning something like this spot is very easy:

Select the clone tool, zoom in nice and close, and select an appropriate
brush.
For this, I used a size 13 brush with a feathered edge, as shown below:

Hit alt, and select an area very close to the area you're going to clone.
For the fiber, I selected a
patch of skin right below the area. Release alt, and then click by click -
DO NOT just paint! -
cover the area you want to clone over. Reselect by clicking alt again if you
need to.

To do the black spot, it's in his hair, so it gets a little tricky. You want
to keep with
the grain of the hair, so I went to the top right a little to keep it going
the same way.

Et voila, both spots are now gone:
